How to Build a PATH

On 13 December 2024, FORMEM concluded the last of five workshops on building a PATH (Planning Alternative Tomorrows with Hope), a tool within the Person-Centred Planning support methodology.
The final session took place at APC Vila Real and was facilitated by professionals from ASSOL. This workshop series was aimed at professionals who support persons with disabilities and who already have prior knowledge of Person-Centred Planning, consolidating and deepening their skills in applying this practical tool.

This workshop series was organised as part of the awareness-raising activities on Person-Centred Planning within the project A Pessoa no Centro dos Apoios, co-financed by the National Institute for Rehabilitation, I.P.
